Nassau County is one of the two counties that make up Long Island. Located immediately east of the borough of Queens in New York City, Nassau is the first county travelers reach when going to Long Island from the city. The Life Plan community in Port Jefferson offers single-level dwellings for independent living, assisted living and memory care along with supportive services without needing to move to a different facility.
